“The Benefits of Collaborating with a Concierge Geriatric Care Manager to Meet the Comprehensive Needs of Your Clients.” 

About the Topic

Assessing clients’ needs, short- and long-term care needs and identifying cost projections to assist with:

  • Financial management

  • Identification of increased care costs including housing modification, along with the potential of the modifications increasing or potentially decreasing the home value.

  • Cost projections if an alternate living arrangement such as an Assisted Living Facility or Skilled Nursing Facility needs to be considered.

  • Assistance in ascertaining if a "younger" disabled client could be eligible for an Achieving a Better Life Experience (ABLE) account

  • Assistance in ascertaining if a client with a long-term care policy is eligible to initiate activation of the policy, along with coordinating the process of the LTC evaluation and advocacy to support clients need for LTC policy activation.

  • Protecting the Elderly from financial abuse:

  • Collaboration with legal and financial advisors on warning signs, identifying plans of action to rectify issues and ensure overall safety of client.

Anne is a concierge certified geriatric care manager who for the past 15 years has provided a comprehensive range of services for senior clients and their families in Westchester County.  Her professional background includes 35 + years as a licensed clinical social worker with a Master’s degree in clinical social work from Simmons College in Boston, MA, and a post-master’s certificate in Gerontology from the Hunter/Brookdale Center on Aging.

She specializes in collaborations with elder law attorneys, guardians, wealth managers and daily money managers, among others, regarding complicated situations related to family conflicts surrounding financial disputes, undue coercion or influence, and suspected issues of elder neglect or financial abuse.  As a GCM, Anne has coordinated with Guardians to provide regular IP care management oversite in between the required four guardian visits per year, including the initiation of in-home services services and medical oversite, and assists in the preparation of the annual Guardianship reports.  Anne also works with solo-aging seniors, and at times will accept the role of primary health care proxy.

Additionally, she has  strong clinical knowledge in mental health issues, and has presented at numerous national conferences on Hoarding Disorder.
